Q. What should I bring with me?

Please print and fill out the appropriate patient forms prior to your appointment. (We will let you know which one when you call for an appointment.) Bring any imaging you have had done previously, which includes MRI and X-ray images.


Q. How do I get a refill of my medication?

You must call your pharmacy and have them fax a request to our office. Our fax number is (801) 314-5112.

MRI & Records

Q. Can I get MRI or lab results explained over the phone?

You must schedule an appointment to have Dr. Bowen, Dr. Hurst, or Dr. Clements explain your results.


Q. Will my insurance cover procedures done in this office?

This is something that varies depending on your provider. Is is your responsibility to contact your insurance provider to inquire about your coverage.

Here is what you should know:

  • Who is your primary insurer?
  • Is Dr. Brent J Bowen, Dr. Hurst, and physician assistant Stephen Clements, P.A.-C on your preferred provider list?
  • Do you need a referral to go to a specialist?
  • How much is your co-pay and deductible?
  • The physician or the physician assistant will not know how much your insurance will cover. Please don't expect them to. Please make sure this is something you know prior to your appointment.
  • Medicaid does not cover the procedures we perform in this office. If your coverage is through Medicaid, we recommend that you seek another provider.

Is Our Practice Right For You?

We diagnose and treat people with numb hands/arms, numb legs/feet, upper, mid, and low back pain, painful arms, legs, and joints. If a medical doctor of another specialty or a colleague of another profession is a better match for your needs, we will get you pointed in the right direction.
